Non-Surgical vs. Surgical Contouring: What’s Right for You?

Comparing CoolSculpting, Kybella, Liposuction, and Fat Transfer

If you’re thinking about body contouring, you’re probably not asking, “What’s the trendiest treatment?” You’re asking, “What will actually work for my body?”

At Illume, we see four options come up again and again because they solve different problems in different ways: CoolSculpting, Kybella, liposuction, and fat transfer. Some are non-surgical and gradual. Some are surgical and more immediate. The best choice comes down to what you’re trying to change, where it’s located, and how quickly you want to see results.

Below is a simple guide to help you compare your options.

If You Want Non-Surgical Contouring

Non-surgical treatments are best when you’re close to your goal weight and dealing with stubborn pockets that don’t respond to healthy habits. These options can be a great fit if you want little downtime and a more gradual change.

CoolSculpting

CoolSculpting is a non-invasive fat reduction treatment that works by freezing fat cells in a targeted area. Over time, your body processes and clears those fat cells, which helps reduce thickness in that spot. Many patients choose it because it’s in-office, non-surgical, and requires no traditional recovery time.

CoolSculpting is commonly used for areas like:

  • abdomen
  • flanks (“love handles”)
  • thighs
  • arms

If your concern is specifically under the chin, Illume also offers CoolMini as a targeted option for that smaller area.

Kybella

Kybella is an injectable treatment designed to improve the appearance of fullness under the chin. It uses a formulation of deoxycholic acid (a substance your body naturally makes) to target and destroy fat cells in the treated area. Once those fat cells are gone, they can’t store fat the same way again.

Kybella is focused and specific, so it’s usually discussed for:

  • the area under the chin (“double chin”)
  • small, localized fullness where the goal is refinement rather than full-body contouring

If You Want Surgical Contouring

Surgical options are typically best when you want a more dramatic change, you have more fat to remove than a non-surgical option can realistically address, or you want precise shaping in multiple areas at once.

Liposuction

Liposuction removes fat through small access points using a thin tube called a cannula. At Illume, we use Microaire’s Power Assisted Liposuction (PAL), which uses powered vibrations to loosen stubborn fat cells and allows for very controlled, precise sculpting.

Liposuction is often a great option when:

  • you want more immediate, noticeable contouring
  • the area is larger than what non-surgical treatments are best suited for
  • you want to treat multiple areas with one plan (like abdomen, waist, and back)

It’s important to know that liposuction is not a skin-tightening procedure. If loose skin is the main concern, your surgeon may talk with you about other options that better match your goals.

Fat Transfer

Fat transfer is a little different, because we’re not removing fat. Instead, we’re moving it. A fat transfer takes fat from an area with excess (often through liposuction), then places it in an area that would benefit from added volume, such as the face, breasts, hands, torso, or buttocks.

Patients love fat transfer because it can create very natural-looking volume using your own tissue, and it often comes with a “two-in-one” benefit: contouring where fat is removed and enhancement where it’s added.

Quick Clarity: Which Option Matches Which Goal?

Here’s the easiest way to think about it:

  • If you want to shrink a stubborn pocket without surgery, start by looking at CoolSculpting.
  • If your main concern is fullness under the chin and you want an injectable approach, Kybella may be worth discussing.
  • If you want more dramatic contouring or more precise shaping, liposuction is often the strongest option.
  • If you want contouring plus added volume somewhere else, fat transfer may be the perfect fit.

Can You Combine Treatments?

Yes, and this is where great results often happen. Many patients use a layered approach, especially when they want refinement in more than one area. For example, liposuction can contour the midsection while fat transfer adds volume to another area. Or a patient may start with CoolSculpting and decide later they want a more dramatic surgical result.

During a consultation, we’ll help you decide what makes the most sense based on your anatomy, skin quality, timeline, and comfort level.

Let’s Choose the Right Option Together

There’s no single “best” contouring treatment. The best option is the one that fits your body and your goals, with realistic expectations from the start. If you’re not sure whether you need non-surgical contouring, surgery, or a combination, that’s exactly what we’re here for.

Book a consultation and we’ll walk you through the options, recommend a plan, and help you feel confident about your next step.
