You can get a full night’s sleep and still feel like your eyes make you look tired. Maybe your upper eyelids have started to droop, or you’ve noticed under-eye bags that never seem to go away. These changes are common as we age, but they can also be influenced by genetics and the natural structure of your face.
At Illume Cosmetic Surgery & MedSpa, eyelid surgery (blepharoplasty) can help address these concerns by removing or repositioning excess skin and fat around the eyes. However, upper and lower blepharoplasty treat different areas and are designed to achieve different results.
Understanding the difference can help you determine which procedure may be right for your goals.
What Is Blepharoplasty?
Blepharoplasty, commonly called an eyelid lift, is a surgical procedure that improves the appearance of the upper eyelids, lower eyelids, or both. It is designed to address concerns such as loose skin, puffiness, and sagging that can make the eyes appear tired or aged.
While some patients benefit from treating only one area, others may choose to combine upper and lower eyelid surgery for a more complete refresh.
The right approach depends on your eyelid anatomy, skin quality, and the specific changes you want to address.
Upper Blepharoplasty: Treating Drooping and Heavy Eyelids
Upper blepharoplasty focuses on the area above the eyes. Over time, the skin of the upper eyelids can lose elasticity, creating folds that hang over the natural eyelid crease. For some people, this makes their eyes appear smaller, heavier, or less defined.
Upper eyelid surgery is commonly used to address:
- Loose or sagging skin on the upper eyelids
- Heavy eyelids that create a tired appearance
- Excess skin that covers the natural eyelid crease
- Puffiness caused by excess fat in the upper eyelids
- Upper eyelid skin that interferes with peripheral vision
During upper blepharoplasty, your surgeon makes an incision within the natural eyelid crease to remove excess skin and, when appropriate, fatty tissue. The incision is positioned to help keep the resulting scar discreet as it heals.
For patients whose upper eyelid skin affects their field of vision, the procedure may offer functional benefits in addition to cosmetic improvement.
It is important to remember that not all upper eyelid drooping comes from excess skin. A low brow or drooping eyelid margin may require a different or additional surgical approach.
Lower Blepharoplasty: Treating Under-Eye Bags and Puffiness
Lower blepharoplasty focuses on the area beneath the eyes. If you constantly notice puffiness or bags under your eyes, even when you’re well-rested, the issue may be related to changes in the fat and skin surrounding your lower eyelids.
As we age, the tissues supporting the lower eyelids can weaken, allowing fat to become more noticeable and the skin to lose firmness.
Lower eyelid surgery can help improve:
- Persistent bags or puffiness beneath the eyes
- Bulging fat deposits under the lower eyelids
- Loose or excess lower eyelid skin
- Certain fine wrinkles associated with excess skin
- An uneven transition between the lower eyelid and cheek caused by prominent fat pockets
Depending on your needs, your surgeon may remove or reposition fat to create a smoother under-eye contour. Excess skin can also be addressed when necessary.
Incisions are typically placed just beneath the lower eyelashes or, in certain cases, inside the lower eyelid.
The goal is to create a smoother, more rested appearance without dramatically changing your natural eye shape.
Upper vs. Lower Blepharoplasty: Which One Do You Need?
The biggest difference between these procedures is the location and cause of your concerns.
If you notice loose skin hanging over your upper lashes, heaviness above your eyes, or difficulty seeing your natural eyelid crease, upper blepharoplasty may be the better option.
If your primary concern is puffiness, under-eye bags, or loose skin beneath the eyes, lower blepharoplasty may be more appropriate.
Some patients experience both concerns. In these cases, upper and lower blepharoplasty can be performed together to address the entire eye area during one surgical plan.
It is also worth knowing what eyelid surgery cannot fully correct. Dark circles caused by pigmentation, crow’s feet, and sagging eyebrows may respond better to other treatments. At Illume, your surgeon can evaluate whether eyelid surgery alone or a combination of procedures will best address your concerns.
What Is Recovery Like After Eyelid Surgery?
Recovery varies depending on whether you undergo upper blepharoplasty, lower blepharoplasty, or both.
At Illume, eyelid surgery is performed in accredited, in-house surgical suites and generally takes approximately one to two hours.
Following surgery, it is normal to experience temporary swelling, bruising, and some discomfort around the eyes. Many patients plan for approximately one to two weeks of initial recovery, although healing times vary and residual swelling can take longer to resolve.
Your surgeon will provide instructions for caring for your incisions, managing swelling, and returning to normal activities. As the area heals, the eyelids gradually become smoother and more defined.
Like any surgical procedure, blepharoplasty carries risks, including bleeding, infection, dry eyes, scarring, changes in eyelid position, and rare vision complications. Your surgeon will review these risks and discuss what to expect during your consultation.
